In this role, you will provide exceptional care to adult and geriatric patients, supporting their health and independence in the comfort of their homes. Position Details:

  • Job Title: Home Health Field RN
  • Location: Amherst, NY, and surrounding areas 
  • Schedule: Full-time, 40 hours per week (Monday-Friday, 8:00 AM - 5:00 PM)
  • On-Call: Typically one weekend per month 
  • Patient Population: Adult and Geriatric
  • Productivity: 25-32 units per week, up to 35 patients per week 
  • Mileage Reimbursement: $0.48 per mile (visit to visit), 150-250 miles per week expected 
  • Benefits: Medical, Dental, Vision, 401(k) after 90 days
  • Pay: Negotiable based on territory budget and candidate experience, $55-$60 per hour, common range negotiated 

    Key Responsibilities:

  • Patient Care: Conduct comprehensive assessments and provide skilled nursing care to patients in their homes, ensuring care plans are executed with precision.
  • OASIS Documentation: Complete OASIS (Outcome and Assessment Information Set) documentation accurately to ensure proper patient care planning and compliance with regulatory standards.
  • Homecare Homebase: Utilize Homecare Homebase (HCHB) software to document patient visits, manage care plans, and communicate with the healthcare team.
  • Coordination: Collaborate with physicians, physical therapists, and other healthcare professionals to deliver coordinated, patient-centered care.
  • On-Call Support: Provide weekend on-call support on a rotational basis, addressing urgent patient needs.
  • Patient Education: Educate patients and their families on disease management, medication administration, and lifestyle modifications to promote optimal health.

    Requirements:

  • Active New York Registered Nurse (RN) License
  • Current Basic Life Support (BLS) certification from the American Heart Association or the American Red Cross
  • Minimum of 1 year of experience as an RN in a home health setting
  • Proficiency in OASIS and Homecare Homebase (HCHB) software required 
  • Oasis Certified strongly preferred. 
  • Reliable transportation and a valid driver’s license
  • Ability to complete Joint Commission onboarding standards
