Job Details:

  • Position: Home Health 
  • Location: Salisbury, NC
  • Type: Full-Time, 40 hours per week
  • Duration: 13-week contract with possibility of extension upon successful completion
  • Schedule: Monday-Friday until shift is complete, one weekend per month on-call
  • Benefits: Mileage reimbursement (.38c visit to visit), Medical, Dental, Vision, 401k eligibility after 90 days, Pay negotiable based off experience and territory, Weekly Pay 
  • Productivity: 30 per week 

    Requirements: 

  • Active North Carolina Licensed Practical Nurse (LPN) License
  • Minimum 1 year of home health experience
  • Proficiency in Homecare Homebase required 
  • Active American Heart Association or American Red Cross BLS
  • Reliable transportation 

    Job Duties:

  • Administer medications and treatments as prescribed, ensuring patient comfort and safety.
  • Monitor and evaluate patients' progress, adjusting care plans as necessary to promote optimal outcomes.
  • Provide education and support to patients and their families on managing health conditions and promoting wellness.
  • Collaborate with interdisciplinary teams to coordinate comprehensive care and facilitate smooth transitions between healthcare settings.
  • Accurately document using Homecare Homebase.
