We’re working with a forward-thinking healthcare organization to place a Family Nurse Practitioner (FNP) in a hybrid home-based and virtual role serving the diverse communities of Queens, NY. This is a 13-week contract position, with 4-week extension options and the potential for permanent placement.

Position Summary:
As part of an integrated care team, the FNP will deliver equitable, value-based care directly in patients’ homes and through telehealth. You'll conduct assessments, provide longitudinal follow-up, manage chronic conditions, and support preventive care, all while forming meaningful relationships with underserved individuals in the community.

Key Responsibilities:

  • Perform in-home and virtual comprehensive patient assessments including Annual Wellness Visits (AWV) and Health Risk Assessments (HRA).

  • Manage chronic and acute conditions, provide follow-up care, and reduce avoidable hospitalizations.

  • Collaborate with interdisciplinary team members to create and implement personalized treatment plans.

  • Prescribe medications and guide lifestyle modifications as needed.

  • Maintain thorough documentation in the EMR system.

  • Participate in quality initiatives and adhere to all state and federal clinical practice guidelines.

  • Occasional on-call support for nurse triage line as needed.

    Schedule & Logistics:

  • Location: Queens, NY, and surrounding areas

  • Schedule: Monday through Friday, 8:00 am – 5:00 pm

  • Travel: Regular in-home visits required (reliable transportation needed)

  • Setting: Hybrid (in-home and telehealth visits)

  • Contract: 13 weeks with option to extend every 4 weeks; possible permanent hire

    Qualifications:

  • Active, unrestricted FNP license in New York 

  • Board Certification (ANCC or AANP)

  • Valid DEA, NPI, BLS (AHA)

  • Experience in primary care, home health, or outpatient settings

  • Ability to treat across the lifespan

  • Reliable transportation and valid auto insurance

  • Proficiency in EMR systems; Medent experience a plus

  • Compassionate, adaptable, and committed to community health

    Preferred:

  • Familiarity with HCC coding, HEDIS, STAR, and Health Risk Assessments

  • Bilingual proficiency

    Compensation & Benefits:

  • Hourly Rate: Negotiable with client; typical range $85–$95/hr
