Contract Home Health RN

Sunbelt is seeking a home health Traveling Home Health Registered Nurse for a position in Cookeville, TN. The ideal candidate must possess strong, patient management, and documentation skills. A minimum of one year of home health RN experience preferred. Must have current RN licensure in state of Virginia, possess a valid driver’s license, vehicle insurance and access to a dependable transportation. Valid BLS certification required.

Additional Details:

• 2 years of Home Health Experience
• Homecare Homebase experience
• Oasis C or D experience or at least Oasis experience required
• Ability to work flexible hours as required to meet identified patient needs.
• Communicates plan of care to patients regularly and provide updates.
• Skilled Nursing Visits/ Admissions/ Starts of Care Experience.
• Maintain accurate, detailed, and timely documentation consistent with the plan of care and the nursing processes
• Ability to travel 75%
• Valid driver's license and reliable transportation; required liability car insurance
