Home Health Registered Nurse (RN) – Contract Opportunity
Location: Hawthorne, NY
Schedule: Monday–Friday, Days
Hours: 40 hours/week
Assignment Length: 13 Weeks
A reputable home health agency is seeking a skilled and compassionate Home Health RN to support patients in the Hawthorne, NY area. This role focuses on delivering quality in-home care while maintaining accurate documentation using OASIS and HomeCare HomeBase (HCHB).
Key Responsibilities:
Provide direct nursing care in the home setting (wound care, med administration, assessments)
Complete OASIS documentation accurately and on time using HCHB
Coordinate with interdisciplinary teams to manage individualized care plans
Educate patients and families on disease management and home care routines
Requirements:
Active NY RN license
At least 1 year of recent home health nursing experience
Proficiency with OASIS & HomeCare HomeBase (HCHB) required
BLS certification (AHA)
Reliable transportation and valid driver’s license
